How to Apply for Financial Assistance to the Recipients of the Service Medal by the Government of Madhya Pradesh

Application mode: Offline

##### *Application Process*

Step 1:**** The interested applicant should visit (during office hours) the District Sainik Welfare Office and request a hard copy of the prescribed format of the application form from the staff exclusively entrusted to issue and collect filled-in applications.

Step 2: In the application form, fill in all the mandatory fields, and attach copies of all the mandatory documents (self-attest, if required).

Step 3: Submit the duly filled and signed application form along with the documents to the District Sainik Welfare Office.

##### *Post-Application Process*

Step 1:**** The District Sainik Welfare Office verifies the application and attached documents.

Step 2:**** The verified application is forwarded to the Directorate of Sainik Welfare, Bhopal, for approval.

Step 3:**** The Directorate sends the approved case to the General Administration Department (GAD-4), Government of Madhya Pradesh, for sanction.

Step 4:**** Upon approval, the payment order is issued to the concerned District Collector.

Step 5:**** The District Collector disburses the sanctioned amount directly into the beneficiary’s bank account.
