How to Apply for Procedure for Making Medical Certificate (for Certifying Disability)
Application mode: Offline
To obtain a medical certificate, a person with disabilities must visit the district hospital on Wednesdays. They can collect a registration form there after paying a nominal fee. After registration, the individual is examined by the concerned doctors or specialists relevant to their type of disability.
The applicant must carry their Aadhaar card, mobile number, and a passport-size photograph. The Chief Medical Officer (CMO) is generally present at the district hospital every Wednesday and, based on the medical examination reports, issues the disability certificate to the concerned individual. If the certificate is not available immediately, it can later be collected from the office of the Chief Medical Officer.
