Unique Disability Identity - DNH & DD
UDID Card or Unique Disability Identity card is an initiative by Government of India with a view of creating a National Database for Persons with Disabilities (PwDs). Under the UDID project Disability Certificate and Unique Disability Identity Card is issued to each PwD through online platform.
About the Unique Disability Identity - DNH & DD Scheme
The “Unique Disability ID (UDID)” has been launched by the Department of Empowerment of Persons with Disabilities (DEPwD), Government of India, aiming to create a national database of Persons with Disabilities (PwDs) and issue a Unique Disability Identity Card to each individual to ensure transparency, efficiency, and ease in delivering government benefits. Through this scheme, Disability Certificates and Unique Disability Identity Cards are provided to Persons with Disabilities across the country. The scheme is implemented by competent medical authorities notified by the respective State Governments and Union Territories through the centralised UDID portal. The project supports online application submission, assessment of disability percentage by hospitals or Medical Boards, online renewal and updating of information, and integrated management of government schemes and benefits for PwDs. It also eliminates duplicate records and enables tracking of the physical and financial progress of beneficiaries at the village, block, district, state, and national levels. District Welfare Officers assist in registration camps and application management, while District Collectors monitor implementation through reporting features available on the portal.

Benefits
The Unique Disability Identity (UDID) card, also known as the Swavlamban Card, is a mandatory document for persons with disabilities (PwDs) in the Union Territory of Dadra & Nagar Haveli and Daman & Diu to access various central and state government welfare schemes. The UDID card ensures a uniform, digitized record that eliminates the need for carrying multiple documents and provides a single, nation-wide proof of disability.

Eligibility Criteria
- The applicant should be suffering from any of the 21 specified disabilities listed under the Rights of Persons with Disabilities Act, 2016.
- The applicant should apply for issuance of a Disability Certificate and Unique Disability Identity (UDID) card through the UDID portal.
- The applicant should undergo disability assessment by the concerned hospital, Medical Board, or competent medical authority for the determination of disability percentage.
- The applicant should get the disability verified and certified by medical authorities notified by the respective State Government or Union Territory Administration.
- The applicant should provide the required personal, medical, and disability-related details during the application process.

How to Apply for Unique Disability Identity - DNH & DD
Application mode: Online
Step 1: Visit the official UDID Portal.
Step 2: Click on “Apply For UDID” on the homepage.
Step 3: Register as a Person with Disability (PwD) on the portal.
Step 4: Log in using the registered credentials.
Step 5: Fill in the online application form for the Disability Certificate and UDID card with the required details.
Step 6: Upload the required documents, including medical and identity-related documents.
Step 7: Submit the application through the portal.
Step 8: The concerned Medical Authority/Medical Board verifies the application and conducts disability assessment for the calculation of disability percentage.
Step 9: The Medical Authority issues the Disability Certificate and UDID card electronically through the portal upon approval.
Step 10: Track the application status using the “Track Your Application” facility available on the portal.
Step 11: Download and print the Disability Certificate and UDID card after approval.
Step 12: Use the portal for renewal, replacement, or updating of information whenever required.
